Eligibility Requirements

Adult Dental Clinic

Patients must be 18 or older, uninsured and live within our service area.

Patients must provide a photo ID and proof of address – utility bill or rent receipt.

Adult Diabetes and Hypertension Screening

Patients must be uninsured, live within our service area and NOT currently under treatment.

Patients must provide a photo ID and proof of address – utility bill or rent receipt.

Back to School

Students must live within our service area.

Parents must provide a photo ID for themselves, proof of address and a social security card or birth certificate for each child.

Children’s Medical Clinic

Patients must be 18 or younger, uninsured and live within our service area.

Parents must provide a photo ID for themselves, proof of address – utility bill or rent receipt - and a social security card or birth certificate for each child.

Clothing

Clients must live within our service area.

Clients must provide a photo ID and proof of address.

Dermatology Clinic

Patients must be uninsured and live within our service area.

Patients must provide a photo ID and proof of address – utility bill or rent receipt.

Evening Medical Clinic

Patients must be uninsured and live within our service area.

Adult patients must provide a photo ID and proof of address – utility bill or rent receipt.

Parents must provide a photo ID for themselves, proof of address – utility bill or rent receipt - and a social security card or birth certificate for each child.

Financial Aid

Clients must live within our service area and have verifiable income.

Clients must provide a photo ID, proof of address, and bring in the rent and/or utility bill that needs to be paid.

Food Pantry

Clients must live within our service area to receive food more than once.

Clients must provide photo ID and proof of address.

Food Stamp Application Assistance

Clients applying for food stamps must provide proof of income.

Applicants must meet income eligibility guidelines set by the federal government and be U.S. citizens.

Friday FoodBasket

Clients must be 60 or older or disabled and referred through the emergency aid center. There is no zip code requirement.

Clients must provide proof of age – photo ID or birth certificate. Annually, they must provide income information – social security award letter, bank statement, pay stubs.

Job Counseling

There is no zip code requirement.  Clients must provide a photo ID and social security card.

Medicaid/CHIP Application Assistance

Clients seeking application assistance must have children no older than age 18. Clients applying for Medicaid must provide proof of income.

Applicants must meet income eligibility guidelines set by the federal government, be U.S. citizens and present a social security card or birth certificate.

PAN (People and Nutrition)

Clients must be 60 or older. There is no zip code requirement.

Clients must provide proof of age – photo ID or birth certificate and meet income eligibility guidelines. Must be certified in January and subsequently recertified every January and July by providing proof of income – social security award letter, bank statement, pay stubs.

Podiatry Clinic

Patients must be uninsured and live within our service area.

Patients must provide a photo ID and proof of address – utility bill or rent receipt

Women’s Clinic

Patients must be 18 or older or emancipated, uninsured and live within our service area. They must present a photo ID and proof of address – utility bill or rent receipt.
